Faculty of Dentistry – Request forms
Below are the downloadable PDF versions of the official request forms which are used to initiate cases with the Study and Examination Committee.
A short rundown of the process:
- Print and fill out the required request form.
- Submit filled out request form to the department in charge of teaching the subject.
If your request is rejected by the department, you can choose to discontinue the request or proceed with it. - Submit the received request form to your registrar at the Directorate of International Studies before the deadline.
- Wait for the decision of the Study and Examination Committee.
The committee may postpone your case due to incorrect/insufficient documents submitted. In such a case, you have 8 days to provide the amended documentation. - Receive official decree.
Request forms
If you wish to get the credits recognised for a subject you have completed during previous studies. The subject must match the syllabus of the subject taught at Semmelweis University by at least 75%.

Required attachments:
- Request form signed by the head of the department in charge of teaching the subject
- Course description of the subject
- Proof of completion of the subject (official transcript preferred)
Under special circumstances, you may request for an exam outside of the exam period which is organised during the first 2 weeks of the term of the following semester.
If multiple subjects are requested, the form must be filled out separately for each. A fee of 7450 HUF per subject must be paid after receiving the official decree and before being able to take the examination. This fee is counted on top of any exam retake fees you may need to pay. Required attachments:
- Proof of illness, death, etc. based on which the permission is requested
Can only be used in case of multi-level subjects which follow eachother, indicated by the roman numerals at the end.
You can request to be able to register for a subject without passing its prerequisite, in case of specific subjects.
If the request is granted, the prerequisite subject should be completed during the exam period of the semester in question, before an exam from the next level of the subject can be attempted. Example: Registering for Macroscopic anatomy and embryology II. without passing Macroscopic anatomy and embryology I.
The failed subject should be registered for on CV course during the registration period for the semester. Please note that you must have remaining exam chances.
If the request is granted, you will be registered for the next level of the subject in your study group, by your registrar. Relevant sections from the Study and Examination Regulations:

Special permission can be used to request a derogation from a provision of the Study and Examination Regulations that does not pertain to academic or financial obligation. Relevant sections from the Study and Examination Regulations:

The EOAY is an examination period organised on the 5 working days following August 20. of every year. During this period, students can take examinations from subjects they have registered for, but haven’t completed during the second semester of the academic year.
In order to be eligible, the application form must be submitted during the application period outlined by the Registrar’s Office, via e-mail, to your registrar. Any applications submitted outside of this time period will not be considered so please be mindful of this.
It is not a requirement to have failed an exam chance during the semester from the subject, to be able to apply for an examination.
