Faculty of Pharmaceutical Sciences – Request forms
Below are the downloadable PDF versions of the official request forms which are used to initiate cases with the Study and Examination Committee.
A short rundown of the process:
- Print and fill out the required request form.

Submit the filled out request form to your registrar at the Directorate of International Studies before the deadline.
If you request for SPECIAL PERMISSION, you have to ask the department in charge of teaching the subject for approval, and after that submit the signed and stamped request form to your registrar. - Wait for the decision of the Study and Examination Committee.
The committee may postpone your case due to incorrect/insufficient documents submitted. In such a case, you have 8 days to provide the amended documentation. - Receive official decree.
Request forms
If you wish to get the credits recognised for a subject you have completed during previous studies. The subject must match the syllabus of the subject taught at Semmelweis University by at least 75%. Relevant sections from the National Higher Education Act:

Required attachments:
- Course description of the subject
- Proof of completion of the subject (official transcript preferred)
Under special circumstances, you may request for an exam outside of the exam period which is organised during the first 2 weeks of the term of the following semester.
If multiple subjects are requested, the form must be filled out separately for each. A fee of 7450 HUF per subject must be paid after receiving the official decree and before being able to take the examination. This fee is counted on top of any exam retake fees you may need to pay. The EOAY is an examination period organised on the 5 working days following August 20. of every year. During this period, students can take examinations from subjects they have registered for, but haven’t completed during the second semester of the academic year.
In order to be eligible, the application form must be submitted during the application period outlined by the Registrar’s Office, via e-mail, to your registrar. Any applications submitted outside of this time period will not be considered so please be mindful of this.
It is not a requirement to have failed an exam chance during the semester from the subject, to be able to apply for an examination.

Special permission can be used to request a derogation from a provision of the Study and Examination Regulations that does not pertain to academic or financial obligation. Relevant sections from the Study and Examination Regulations:

At the Faculty of Pharmaceutical Sciences, a separate examination period is announced for the students whose special permission request has been approved in the last 9 weeks of the term in each semester. (In the autumn semester between October and December; in the spring semester between March and May.)
In case the prerequisite subject is not passed within the examination period announced in the last 9 weeks of the term, then the upper semester subject(s) will get deleted. While the special permission request remains used.
Those who pass the prerequisite subject in the examination period announced in the last 9 weeks of term have to provide the Department(s) responsible for the upper semester subject(s) with a certificate to be eligible for the practical grade of the upper semester subject, or to be eligible to register for the upper semester subject(s)’ exams announced in the regular examination period. Do not forget to notify your Registrars as well about passing the prerequisite subject.
Such a request can only be requested once during your studies and is separate from any other special permission chance.
If the request is granted, you will be registered for the subjects it was prerequisite for in your study group, by your registrar.
