In order to receive a certicate about TDK work, one must be a registered member of the Association for at least 6 months.
To claim a certificate: download and fill this form, then send it to office.se.tdk@gmail.com
The official certificate can be collected 3 workdays later from the TDK office (1082. Budapest, Üllői út 78. – building of the 1st Dept. of Surgery)

To be a member of Semmelweis University Students’ Scientific Association while pursuing TDK work in a non-University institute, it is important for the student to submit this form filled at the start of every semester to the Council.
