All fields in the application form must be completed, and a completed draft of the Learning Agreement for Traineeships must also be attached. This allows the clinic to see what kind of professional activities the student would like to undertake during the traineeship.<\/em><br \/>\n<em>If the form is incomplete or not filled in properly, the application will be considered invalid. In such cases, the Mobility Office will not send a separate notification.<\/em><br \/>\n<em><strong>It is the student\u2019s responsibility to make sure that all application requirements are met and that the form is completed correctly. To do this, your Semmelweis email address must already be active <em>(you should have activated it when you received the notification about your account)<\/em>. Please bring the Helth Booklet we gave you to the examination.<\/li>\n<\/ul>\n<p><em><span style=\"color: #ff0000\"><strong>Important!<\/strong><\/span><\/em><br \/>\n<em>The medical examination must be completed before you start your clinical traineeship. Without it, you are not allowed to enter the hospital or take part in patient care. If you do not complete the required examination, we will not be able to issue your final mobility documents at the end of your stay.<\/em><\/p>\n<ul>\n<li>If you would like to request a temporary
