Absztrakt<\/strong><\/p>\n A palliat\u00edv ell\u00e1t\u00e1sok korai bevon\u00e1sa az onkol\u00f3giai betegek ell\u00e1t\u00e1s\u00e1ba el\u0151nnyel j\u00e1r mind a beteg \u00e9letmin\u0151s\u00e9ge, mind a kezel\u00e9s sz\u00ednvonala, mind a k\u00f6lts\u00e9ghat\u00e9konys\u00e1g szempontj\u00e1b\u00f3l. Sz\u00fcks\u00e9g van egy olyan modellre, mely alapj\u00e1n a meg\u00adfelel\u0151 id\u0151ben, <\/em>a megfelel\u0151 beteg <\/em>a megfelel\u0151 min\u0151s\u00e9g\u0171 palliat\u00edv <\/em>ell\u00e1t\u00e1sban r\u00e9szes\u00fcl. Ebben a k\u00f6zlem\u00e9nyben a palliat\u00edv el\u00adl\u00e1t\u00e1s korai integr\u00e1ci\u00f3j\u00e1nak el\u0151nyei, szintjei \u00e9s a speci\u00e1lis palliat\u00edv ell\u00e1t\u00e1s fogalm\u00e1nak ismertet\u00e9se ut\u00e1n a legink\u00e1bb elter\u00adjedt beutal\u00e1si modellek el\u0151nyeit \u00e9s h\u00e1tr\u00e1nyait mutatjuk be a nemzetk\u00f6zi szakirodalom alapj\u00e1n. A speci\u00e1lis palliat\u00edv ell\u00e1t\u00e1st ig\u00e9nyl\u0151 betegek kisz\u0171r\u00e9s\u00e9re szolg\u00e1l\u00f3, progn\u00f3zisalap\u00fa modellek h\u00e1tr\u00e1nya, hogy nincs el\u00e9g kapacit\u00e1s az ilyen m\u00f3don beutalt nagysz\u00e1m\u00fa beteg ell\u00e1t\u00e1s\u00e1ra, ez\u00e9rt sz\u00e9les k\u00f6rben nem terjedtek el. A t\u00fcneteken alapul\u00f3 modellek sok\u00adszor bonyolultak \u00e9s a mindennapi klinikai gyakorlatban nem haszn\u00e1latosak. Az \u00faj kutat\u00e1sok alapj\u00e1n egyszer\u0171, gyakor\u00adlatias k\u00e9rd\u00e9ssorokat alkalmaznak, melyekkel k\u00f6nnyen kiemelhet\u0151k, akik profit\u00e1lnak a speci\u00e1lis palliat\u00edv intervenci\u00f3b\u00f3l. Ezek k\u00f6z\u00fcl a Yale egyetemi \u00e9s a PALLIA -10 modellt ismertetj\u00fck r\u00e9szletesen. Amennyiben az akt\u00edv onkol\u00f3giai ell\u00e1t\u00e1st v\u00e9gz\u0151k a megfelel\u0151 palliat\u00edv beutal\u00e1si krit\u00e9riumokat ismerik \u00e9s alkalmazz\u00e1k, a betegek id\u0151ben jutnak a megfelel\u0151 komplex kezel\u00e9shez an\u00e9lk\u00fcl, hogy a palliat\u00edv ell\u00e1t\u00f3rendszer t\u00falterhel\u0151dne.<\/p>\n Kulcsszavak: <\/strong>hospice, palliat\u00edv ell\u00e1t\u00e1s, onkol\u00f3gia, integr\u00e1ci\u00f3, beutal\u00e1s, ir\u00e1ny\u00edt\u00e1s<\/p>\n <\/p>\n When and how to integrate oncology and palliative care? <\/strong><\/p>\n Abstract<\/strong><\/p>\n Early integration of palliative care into the trajectory of cancer care brings advantages into the patients\u2019 quality of life, the level of care and cost-efficiency, too. On the basis of a predefined model, the right patient <\/em>may receive the right level of palliative care at the right time<\/em>. Having defined the advantages, the levels of early integration of palliative care and the concept of special palliative care, we also aim to describe the advantages and disadvantages of the most com\u00admon referral models on the basis of international literature in this article. The drawback of prognosis-based models to identify patients needing special palliative care is the lack of capacity to provide care for the large number of pa\u00adtients so recognised; therefore they have not become widespread. Needs-based models tend to be complicated and thus rarely applied in everyday clinical practice. On the basis of new researches, simple, pragmatic questionnaires are utilised through which the patients who could benefit from special palliative care interventions are easy to identify. Here we give a detailed report of the Yale University and PALLIA-10 models.
