Keresztmetszeti vizsg\u00e1latunkban azt elemezt\u00fck hogy az IBD-ben szenved\u0151 gyermekek t\u00f6bb ultrafeldolgozott \u00e9lelmiszert (UPF) fogyasztanak-e, mint az eg\u00e9szs\u00e9ges gyermekek.<\/p>\n<p>A Heim P\u00e1l Orsz\u00e1gos Gyermekgy\u00f3gy\u00e1szati Int\u00e9zet Gasztroenterol\u00f3giai Ambulanci\u00e1j\u00e1n 2023 decembere \u00e9s 2025 febru\u00e1rja k\u00f6z\u00f6tt megjelent IBD-s gyermekek \u00e9tkez\u00e9si szok\u00e1sait hasonl\u00edtottuk \u00f6ssze budapesti iskol\u00e1kban tanul\u00f3 eg\u00e9szs\u00e9ges gyermekek \u00e9tkez\u00e9si szok\u00e1saival. Az UPF bevitelt a NOVA rendszer alapj\u00e1n kategoriz\u00e1ltuk, mely1-t\u0151l 4-ig osztja be az \u00e9lelmiszereket ipari feldolgozotts\u00e1guk szerint, majd az \u00f6sszenergia bevitel sz\u00e1zal\u00e9kak\u00e9nt fejezt\u00fck ki. \u00d6sszesen 47 IBD-s gyermeket hasonl\u00edtottunk \u00f6ssze 47 eg\u00e9szs\u00e9ges gyermekkel. A k\u00e9t csoport k\u00f6z\u00f6tt nem volt k\u00fcl\u00f6nbs\u00e9g a teljes energiabevitel tekintet\u00e9ben, azonban az IBD-s gyermekek szignifik\u00e1nsan t\u00f6bb UPF-et fogyasztottak az eg\u00e9szs\u00e9ges gyermekekhez (50 sz\u00e1zal\u00e9k vs. 40 sz\u00e1zal\u00e9k) k\u00e9pest. Az akt\u00edv betegs\u00e9gben szenved\u0151 gyermekek \u00e9s a remisszi\u00f3ban l\u00e9v\u0151k k\u00f6z\u00f6tt nem volt k\u00fcl\u00f6nbs\u00e9g az UPF-bevitelben, azonban a biol\u00f3giai ter\u00e1pi\u00e1ban r\u00e9szes\u00fcl\u0151 gyermekek szignifik\u00e1nsan kevesebb UPF-et fogyasztottak, mint a hagyom\u00e1nyos kezel\u00e9sben r\u00e9szes\u00fcl\u0151 gyermekek (46 sz\u00e1zal\u00e9k vs. 54 sz\u00e1zal\u00e9k), ami azt mutatja, hogy a s\u00falyosabb betegs\u00e9get tapasztal\u00f3 betegek jobban figyelnek a t\u00e1pl\u00e1lkoz\u00e1sukra \u2013 \u00edrta \u00f6sszefoglal\u00f3j\u00e1ban dr. A D-vitamin-hi\u00e1ny megsz\u00fcntet\u00e9s\u00e9t k\u00f6vet\u0151en, a p\u00f3tl\u00e1s hat\u00e1s\u00e1ra a perif\u00e9ri\u00e1s mononukle\u00e1ris sejtekben cs\u00f6kkent a DNS k\u00e1rosod\u00e1s m\u00e9rt\u00e9ke, a single-cell ATAC-seq elemz\u00e9s alapj\u00e1n n\u0151tt a perif\u00e9ri\u00e1n a monocit\u00e1k \u00e9s az NK-sejtek ar\u00e1nya, megv\u00e1ltozott t\u00f6bb immunsejt kromatin hozz\u00e1f\u00e9rhet\u0151s\u00e9ge \u00e9s g\u00e9nszab\u00e1lyoz\u00e1si mint\u00e1zata. A HANO t\u00fcnetei gyakran m\u00e1r gyermekkorban jelentkeznek, azonban a gyermek nem csup\u00e1n kis feln\u0151tt, \u00edgy ell\u00e1t\u00e1suk speci\u00e1lis megfontol\u00e1sokat ig\u00e9nyel. Ugyanakkor eddig erre a korcsoportra c\u00e9lzott szakmai ir\u00e1nyelv nem \u00e1llt rendelkez\u00e9sre.<\/p>\n<p>Az ir\u00e1nyelv kifejleszt\u00e9s\u00e9ben \u00e9s az \u00f6sszesen 47 aj\u00e1nl\u00e1s kidolgoz\u00e1s\u00e1ban egy 32 f\u0151s, 18 orsz\u00e1gb\u00f3l \u00e9rkez\u0151 szak\u00e9rt\u0151i csapat vett r\u00e9szt, majd az aj\u00e1nl\u00e1sokat a Budapesten tartott 14. C1-inhibitor Deficiency &amp; Angioedema Workshop-on r\u00e9sztvev\u0151 135 eg\u00e9szs\u00e9g\u00fcgyi szakember szavazta meg. A szakmai ir\u00e1nyelv kit\u00e9r a diagnosztika, a kezel\u00e9s, valamint a betegek nyomon k\u00f6vet\u00e9s\u00e9nek r\u00e9szleteire, emellett k\u00fcl\u00f6n hangs\u00falyt fektet azokra a r\u00e9gi\u00f3kra, ahol az els\u0151k\u00e9nt aj\u00e1nlott gy\u00f3gyszerek nem vagy nehezen \u00e9rhet\u0151k el. Ezt a jelens\u00e9get a szakirodalom intraindividu\u00e1lis variabilit\u00e1snak nevezi, amely egyes neuropszichol\u00f3giai popul\u00e1ci\u00f3kban, p\u00e9ld\u00e1ul demenci\u00e1ban, j\u00f3l ismert, stroke ut\u00e1ni af\u00e1zi\u00e1ban azonban m\u00e9g viszonylag keveset vizsg\u00e1lt.<\/p>\n<p>Kutat\u00e1sunkban els\u0151k\u00e9nt vizsg\u00e1ltuk szisztematikusan a nyelvi teljes\u00edtm\u00e9ny ingadoz\u00e1s\u00e1t ebben a betegcsoportban. A t\u00f6bb mint 600 beteg adatait feldolgoz\u00f3 kutat\u00e1s az eddigi legnagyobb betegsz\u00e1m\u00fa gy\u00f3gyszergy\u00e1rt\u00f3t\u00f3l f\u00fcggetlen vizsg\u00e1lat Magyarorsz\u00e1gon.<\/p>\n<p>A vizsg\u00e1lat sor\u00e1n a betegeket k\u00e9t csoportra osztottuk: az egyik csoport m\u00e1r a betegs\u00e9g elej\u00e9n magas energiatartalm\u00fa t\u00e1pl\u00e1l\u00e1st kapott, m\u00edg a m\u00e1sikn\u00e1l a t\u00e1panyagbevitelt fokozatosan, l\u00e9p\u00e9sr\u0151l l\u00e9p\u00e9sre n\u00f6velt\u00fck. A kutat\u00f3k els\u0151dleges c\u00e9lja annak meghat\u00e1roz\u00e1sa volt, hogy melyik m\u00f3dszer mellett alakul ki ritk\u00e1bban s\u00falyos betegs\u00e9g vagy hal\u00e1loz\u00e1s. Az eredm\u00e9nyek alapj\u00e1n a korai, magas kal\u00f3riatartalm\u00fa t\u00e1pl\u00e1l\u00e1s nem j\u00e1rt el\u0151nnyel, s\u0151t egyes esetekben t\u00f6bb sz\u00f6v\u0151dm\u00e9nnyel t\u00e1rsult. Ezzel szemben a fokozatos t\u00e1pl\u00e1l\u00e1s biztons\u00e1gosabbnak bizonyult, \u00e9s nem n\u00f6velte a s\u00falyos \u00e1llapot vagy a hal\u00e1loz\u00e1s kock\u00e1zat\u00e1t. \u00d6sszess\u00e9g\u00e9ben a vizsg\u00e1lat azt mutatta, hogy b\u00e1r kor\u00e1bban sokan \u00fagy v\u00e9lt\u00e9k, hogy a korai, intenz\u00edv t\u00e1pl\u00e1l\u00e1s seg\u00edti a gy\u00f3gyul\u00e1st, akut hasny\u00e1lmirigy-gyullad\u00e1s eset\u00e9n c\u00e9lszer\u0171bb a t\u00e1panyagbevitelt \u00f3vatosan, fokozatosan emelni.
