Tudom\u00e1nyos kutat\u00e1sunkban azt vizsg\u00e1ltuk, hogy a pszichi\u00e1terek hogy viszonyulnak \u00f6nbevall\u00e1suk alapj\u00e1n a ment\u00e1lis betegs\u00e9ggel \u00e9l\u0151 emberekhez, illetve hozz\u00e1\u00e1ll\u00e1sukra milyen t\u00e9nyez\u0151k vannak hat\u00e1ssal. Eur\u00f3pa 32 orsz\u00e1g\u00e1ban folyt a vizsg\u00e1lat. K\u00e9rd\u0151\u00edv\u00fcnket \u00f6sszesen 4245 f\u0151 rezidensek \u00e9s szakorvos t\u00f6lt\u00f6tte ki a gyermek \u00e9s ifj\u00fas\u00e1gpszichi\u00e1tria, valamint a feln\u0151tt pszichi\u00e1tria ter\u00fclet\u00e9r\u0151l.<\/div>\n<div>\u00a0<\/div>\n<div>A vizsg\u00e1latb\u00f3l egyr\u00e9szt kider\u00fclt, hogy a r\u00e9sztvev\u0151k k\u00f6zel k\u00e9tharmad\u00e1nak (63%) akad k\u00f6zeli ismer\u0151sei, vagy csal\u00e1dtagjai k\u00f6z\u00f6tt ment\u00e1lis betegs\u00e9ggel \u00e9l\u0151 szem\u00e9ly, tov\u00e1bb\u00e1 39%-uk ker\u00fclt m\u00e1r olyan helyzetbe, hogy saj\u00e1t ment\u00e1lis \u00e1llapota miatt k\u00e9rt seg\u00edts\u00e9get az ell\u00e1t\u00f3rendszerben.<br \/>\nEredm\u00e9nyeink azt mutatj\u00e1k, hogy azok, akik pszichoter\u00e1pi\u00e1s tev\u00e9kenys\u00e9get folytatnak, esetmegbesz\u00e9l\u0151, csoportokra j\u00e1rnak, illetve akiknek k\u00f6zvetlen munkat\u00e1rsaira nem jellemz\u0151 a p\u00e1cienseket megb\u00e9lyegz\u0151 attit\u0171d, azok \u00f6sszess\u00e9g\u00e9ben kedvez\u0151bb hozz\u00e1\u00e1ll\u00e1st tan\u00fas\u00edtanak a ment\u00e1lis betegs\u00e9ggel \u00e9l\u0151k ir\u00e1ny\u00e1ban &#8211; A felhaszn\u00e1lt, nemzetk\u00f6zileg valid\u00e1lt k\u00e9rd\u0151\u00edvekkel US Department of Agriculture Food Security Survey Module a t\u00e1pl\u00e1lkoz\u00e1si szok\u00e1saikra, m\u00edg a Depression, Anxiety and Stress Scale (DASS-21) lelki, ment\u00e1lis \u00e1llapotukra k\u00e9rdezt\u00fcnk r\u00e1.<\/p>\n<p>\u00d6sszesen 400 v\u00e1laszt \u00e9rt\u00e9kelt\u00fcnk. A szorong\u00e1s volt a leggyakoribb (49%), ezt k\u00f6vette a depresszi\u00f3 (42%) \u00e9s a stressz (30%), mindegyik nagyobb es\u00e9llyel a n\u0151kn\u00e9l. Depresszi\u00f3s t\u00fcnetek f\u0151leg az id\u0151sebbek (25-29 \u00e9v) k\u00f6r\u00e9ben jelentkeztek, akiknek nem volt p\u00e1rtkapcsolatuk, rosszabb eg\u00e9szs\u00e9gi \u00e1llapotban \u00e9rezt\u00e9k magukat, szobat\u00e1rssal laktak. A v\u00e1ltoz\u00f3kor emellett kellemetlen t\u00fcnetekkel j\u00e1r, mint h\u0151hull\u00e1mok, \u00e9jszakai izzad\u00e1s, fejf\u00e1j\u00e1s \u00e9s \u00edz\u00fcleti f\u00e1jdalom. A t\u00fcnetek intenzit\u00e1sa, gyakoris\u00e1ga \u00e9s a kr\u00f3nikus betegs\u00e9gek kialakul\u00e1s\u00e1nak kock\u00e1zata \u00e9letm\u00f3dv\u00e1lt\u00e1ssal befoly\u00e1solhat\u00f3.<\/p>\n<p>A perimenopauza \u00e9trendj\u00e9ben a kiegyens\u00falyozott t\u00e1pl\u00e1lkoz\u00e1s egyes aj\u00e1nl\u00e1sai hangs\u00falyosabbak, javasolt kevesebb s\u00f3 fogyaszt\u00e1s, a feh\u00e9rjebevitel cs\u00f6kkent\u00e9se \u00e9s a n\u00f6v\u00e9nyi feh\u00e9rjeforr\u00e1sok el\u0151t\u00e9rbe helyez\u00e9se, a z\u00f6lds\u00e9g- \u00e9s gy\u00fcm\u00f6lcs fogyaszt\u00e1s n\u00f6vel\u00e9se. B\u00e1r a KRAS aktiv\u00e1l\u00f3 mut\u00e1ci\u00f3j\u00e1nak jelent\u0151s\u00e9ge \u00e9vtizedek \u00f3ta ismert, a mut\u00e1ns KRAS g\u00e1tl\u00e1sa \u00e9vtizedek \u00f3ta kih\u00edv\u00e1s el\u00e9 \u00e1ll\u00edtotta a r\u00e1kkutat\u00e1st. Mivel a kompakt feh\u00e9rj\u00e9n \u00fagy t\u0171nt, nem lehet \u201efog\u00e1st\u201d tal\u00e1lni, k\u00f6zvetett g\u00e1tl\u00e1si m\u00f3dszerekkel pr\u00f3b\u00e1lkoztak a kutat\u00f3k. A KRAS megfelel\u0151 m\u0171k\u00f6d\u00e9s\u00e9hez n\u00e9lk\u00fcl\u00f6zhetetlen a feh\u00e9rje farnezilhorgonnyal t\u00f6rt\u00e9n\u0151 sejtmembr\u00e1nhoz kapcsol\u00f3d\u00e1sa, \u00edgy az egyik leg\u00edg\u00e9retesebb pr\u00f3b\u00e1lkoz\u00e1snak az ezt g\u00e1tl\u00f3 farnezil-transzfer\u00e1z inhibitorok kifejleszt\u00e9se t\u0171nt. Azonban a sikertelen klinikai tesztek sor\u00e1n kider\u00fclt, hogy a KRAS a farnezil-transzfer\u00e1z inhibitorok jelenl\u00e9t\u00e9ben k\u00e9pes egy alternat\u00edv membr\u00e1nhorgonnyal plazmamembr\u00e1nhoz kapcsol\u00f3dni \u00e9s bet\u00f6lteni a tumort seg\u00edt\u0151 funkci\u00f3j\u00e1t. A kudarc ut\u00e1n eg\u00e9szen az elm\u00falt \u00e9vekig a KRAS c\u00e9lzott, k\u00f6zvetlen g\u00e1tl\u00e1sa lehetetlennek t\u0171nt. A k\u00f6zelm\u00faltban ugyanakkor \u2013 \u00e1tt\u00f6r\u00e9st hozva a KRAS g\u00e1tl\u00e1s\u00e1ban &#8211; sikeresen fejlesztettek specifikus inhibitorokat a mut\u00e1ns KRAS egyik t\u00edpus\u00e1ra, a KRAS G12C mut\u00e1ci\u00f3ra. Az \u00fajonnan kifejlesztett inhibitorok egy r\u00e9sz\u00e9t mostanra m\u00e1r enged\u00e9lyeztek is a KRAS-G12C mut\u00e1ns t\u00fcd\u0151r\u00e1k kezel\u00e9s\u00e9re. Ugyanakkor a tumorokban eleve jelen l\u00e9v\u0151, valamint a kezel\u00e9s sor\u00e1n kialakul\u00f3 szerzett rezisztencia miatt csak a betegek kisebb csoportja reag\u00e1l a kezel\u00e9sre, \u00edgy az \u00faj g\u00e1tl\u00f3szerek hat\u00e9konys\u00e1g\u00e1nak n\u00f6vel\u00e9s\u00e9re jelenleg sz\u00e1mos kombin\u00e1ci\u00f3s kezel\u00e9st vizsg\u00e1lnak klinikai tesztek sor\u00e1n.<\/p>\n<p>Munkacsoportunk a RAS feh\u00e9rj\u00e9k membr\u00e1nhorgonyz\u00f3d\u00e1s\u00e1t c\u00e9lz\u00f3 prenil\u00e1ci\u00f3-g\u00e1tl\u00f3szerek vizsg\u00e1lata sor\u00e1n figyelt fel arra a \u2013 fentebb v\u00e1zolt el\u0151zm\u00e9nyek f\u00e9ny\u00e9ben meglep\u0151 \u2013 t\u00e9nyre, hogy a KRAS G12C mut\u00e1ci\u00f3t hordoz\u00f3 sejtvonalak fokozott \u00e9rz\u00e9kenys\u00e9get mutattak a farnezil-transzfer\u00e1z inhibitorokra. Ezen eredm\u00e9ny\u00fcnk nyom\u00e1n kezdt\u00fck vizsg\u00e1lni az \u00faj, klinikai haszn\u00e1latban l\u00e9v\u0151 KRAS G12C inhibitorok kombin\u00e1ci\u00f3j\u00e1t a farnezil-transzfer\u00e1z inhibitorokkal. Vizsg\u00e1lataink sor\u00e1n a kombin\u00e1ci\u00f3s kezel\u00e9s KRAS G12C mut\u00e1ci\u00f3t hordoz\u00f3 t\u00fcd\u0151, kolorekt\u00e1lis \u00e9s hasny\u00e1lmirigyr\u00e1k tumoros sejtvonalak teny\u00e9szetein szinergista, er\u0151teljes tumorellenes hat\u00e1st mutatott. Mindazon\u00e1ltal az elm\u00falt \u00e9vek diagnosztikai fejl\u0151d\u00e9s\u00e9nek eredm\u00e9nyek\u00e9ppen, prosztatar\u00e1k gyan\u00faja eset\u00e9n egyre gyakrabban ker\u00fcl sor MRI-vizsg\u00e1latra a mintav\u00e9telt megel\u0151z\u0151en, valamint MRI-vez\u00e9relt biopszi\u00e1ra.<\/p>\n<p>A szerz\u0151k 12 tanulm\u00e1ny \u00f6sszesen 80 114 sz\u0171r\u00e9sben r\u00e9sztvev\u0151 f\u00e9rfi adat\u00e1t \u00e9rt\u00e9kelt\u00e9k ki, \u00e9s hasonl\u00edtott\u00e1k \u00f6ssze a hagyom\u00e1nyos, kiz\u00e1r\u00f3lag PSA-alap\u00fa \u00e9s az MRI-vizsg\u00e1latot haszn\u00e1l\u00f3 sz\u0171r\u0151vizsg\u00e1latok hat\u00e9konys\u00e1g\u00e1t. Az eredm\u00e9nyek azt mutatj\u00e1k, hogy prosztata-MRI haszn\u00e1lat\u00e1val harmad\u00e1ra cs\u00f6kkenthet\u0151 mind a mintav\u00e9telek, mind a kev\u00e9sb\u00e9 agressz\u00edv, azonnali akt\u00edv kezel\u00e9st nem ig\u00e9nyl\u0151 prosztatadaganatok sz\u00e1ma a kiz\u00e1r\u00f3lag PSA-alap\u00fa sz\u0171r\u00e9shez k\u00e9pest. Mindemellett az MRI seg\u00edts\u00e9g\u00e9vel v\u00e9gzett biopszia n\u00e9gyszer pontosabban k\u00e9pes megtal\u00e1lni a klinikailag agressz\u00edv daganatokat.
