Er\u0151s bizony\u00edt\u00e9kok sz\u00f3lnak amellett, hogy a l\u00edtium, azon t\u00fal, hogy a hangulatbetegs\u00e9gek t\u00fcneteit cs\u00f6kkenti, er\u0151s v\u00e9delmet biztos\u00edt az \u00f6ngyilkos magatart\u00e1ssal szemben ezen betegekn\u00e9l. A Li nyomelemk\u00e9nt az iv\u00f3v\u00edzben is jelen van, azonban t\u00edpusosan olyan alacsony koncentr\u00e1ci\u00f3ban, hogy naponta t\u00f6bb t\u00edzezer liternyi csapvizet kellene elfogyasztanunk ahhoz, hogy az orvosi alkalmaz\u00e1s sor\u00e1n meg\u00e1llap\u00edtott d\u00f3zist vigy\u00fck be. Ennek ellen\u00e9re sz\u00e1mos, k\u00fcl\u00f6nb\u00f6z\u0151 orsz\u00e1gokban kivitelezett, vizsg\u00e1lat szerint, az iv\u00f3v\u00edzben l\u00e9v\u0151 minim\u00e1lis Li mennyis\u00e9g is m\u00e9rs\u00e9kelni k\u00e9pes az \u00e1tlag popul\u00e1ci\u00f3 \u00f6ngyilkoss\u00e1gi kock\u00e1zat\u00e1t.<\/p>\n<p>Vizsg\u00e1latunkban els\u0151k\u00e9nt siker\u00fclt igazolnunk, hogy ez a hat\u00e1s haz\u00e1nkban is kimutathat\u00f3 (a vizsg\u00e1latot j\u00e1r\u00e1si szintre aggreg\u00e1lt adatokon v\u00e9gezt\u00fck el). Az eredm\u00e9nyek szerint, a Li szuicidprevenci\u00f3s hat\u00e1sa els\u0151sorban a f\u00e9rfiak k\u00f6zt mutathat\u00f3 ki, mely val\u00f3sz\u00edn\u0171leg arra vezethet\u0151 vissza, hogy \u0151k gyakrabban v\u00e1lasztanak violens elk\u00f6vet\u00e9si m\u00f3dokat, mint a n\u0151k, \u00e9s a Li antiagressz\u00edv, illetve antiimpulz\u00edv hat\u00e1sa els\u0151sorban ezeket a hal\u00e1leseteket k\u00e9pes kiv\u00e9deni \u2013 fogalmazott \u00f6sszefoglal\u00f3j\u00e1ban dr. Oper\u00e1ci\u00f3 ut\u00e1n ugyanis a betegek egy r\u00e9sze k\u00f6zepes vagy s\u00falyos f\u00e1jdalmat \u00e9l \u00e1t, amely korl\u00e1tozhatja a mindennapi tev\u00e9kenys\u00e9gek elv\u00e9gz\u00e9s\u00e9ben \u00e9s szorong\u00e1st, f\u00e9lelmet, lehangolts\u00e1got v\u00e1lthat ki, h\u00e1tr\u00e1ltatva ezzel a gy\u00f3gyul\u00e1s folyamat\u00e1t. A t\u00f6bb szakter\u00fclet k\u00e9pvisel\u0151it magukban foglal\u00f3 f\u00e1jdalomkezel\u0151 szolg\u00e1latok (Acute Pain Service, APS) olyan \u00faj ell\u00e1t\u00e1si form\u00e1t jelentenek a k\u00f3rh\u00e1zakban, amelyek seg\u00edtik a betegek f\u00e1jdalm\u00e1nak \u00e9s a kezel\u00e9ssel \u00f6sszef\u00fcgg\u0151 mell\u00e9khat\u00e1soknak a cs\u00f6kken\u00e9s\u00e9t, n\u00f6velik a betegbiztons\u00e1got \u00e9s betegel\u00e9gedetts\u00e9get. A kutat\u00e1s az APS magyarorsz\u00e1gi elterjedts\u00e9g\u00e9t, jellemz\u0151it, oktat\u00e1si \u00e9s min\u0151s\u00e9g\u00fcgyi tev\u00e9kenys\u00e9g\u00e9t, valamint a kialak\u00edt\u00e1sukat akad\u00e1lyoz\u00f3 t\u00e9nyez\u0151ket vizsg\u00e1lta.<\/p>\n<p>Jelenleg mind\u00f6ssze k\u00e9t k\u00f3rh\u00e1z m\u0171k\u00f6dtet f\u00e1jdalomkezel\u0151 csapatot, amelyek k\u00f6z\u00fcl az egyik orvosokb\u00f3l, a m\u00e1sik pedig aneszteziol\u00f3gus \u00e1ltal fel\u00fcgyelt eg\u00e9szs\u00e9g\u00fcgyi szakdolgoz\u00f3kb\u00f3l \u00e1ll. Tev\u00e9kenys\u00e9g\u00fck megfelel a szakirodalomban fellelhet\u0151 elv\u00e1r\u00e1soknak, min\u0151s\u00e9gi krit\u00e9riumoknak. Azokban a k\u00f3rh\u00e1zakban is elismerik a szolg\u00e1latok l\u00e9trehoz\u00e1s\u00e1nak sz\u00fcks\u00e9gess\u00e9g\u00e9t, ahol nincs APS. A coronaria plakk progresszi\u00f3 a kardiovaszkul\u00e1ris esem\u00e9nyek f\u00fcggetlen prediktora, azonban defin\u00edci\u00f3ja nem egys\u00e9ges. C\u00e9lunk a koszor\u00fa\u00e9r-betegs\u00e9g s\u00falyoss\u00e1g\u00e1ra \u00e9s kiterjed\u00e9s\u00e9re jelenleg alkalmazott k\u00fcl\u00f6nb\u00f6z\u0151 defin\u00edci\u00f3k szerint a plakk-progresszi\u00f3 meghat\u00e1roz\u00e1sa volt. Emellett elemezt\u00fck, hogy mely rizik\u00f3t\u00e9nyez\u0151k befoly\u00e1solj\u00e1k a koszor\u00fa\u00e9r-betegs\u00e9g progresszi\u00f3j\u00e1t. \u00d6sszesen 115 stabil angin\u00e1s beteg coronaria CTA felv\u00e9tel\u00e9t vizsg\u00e1ltuk a V\u00e1rosmajori Sz\u00edv- \u00e9s \u00c9rgy\u00f3gy\u00e1szati Klinik\u00e1n.<\/p>\n<p>Eredm\u00e9nyeink alapj\u00e1n a k\u00fcl\u00f6nb\u00f6z\u0151 defin\u00edci\u00f3k alkalmaz\u00e1s\u00e1val jelent\u0151sen elt\u00e9r\u0151 m\u00e9rt\u00e9k\u0171 progresszi\u00f3t detekt\u00e1ltunk. Kiemelend\u0151, hogy a klinikumban haszn\u00e1lt \u00fan. CAD-RADS (coronary artery disease \u2013 reporting and data system) oszt\u00e1lyoz\u00e1s az esetek t\u00f6bb mint fel\u00e9ben nem azonos\u00edtotta a plakkok progresszi\u00f3j\u00e1t m\u00e1s defin\u00edci\u00f3kkal ellent\u00e9tben. Ezen fel\u00fcl a diabetes, a doh\u00e1nyz\u00e1s, a f\u00e9rfi nem \u00e9s az \u00e9letkor befoly\u00e1solta szignifik\u00e1ns m\u00e9rt\u00e9kben a coronaria plakkok progresszi\u00f3j\u00e1t \u2013 foglalta \u00f6ssze dr. A h\u00e1zi sert\u00e9sek a jelens\u00e9g term\u00e9szetes t\u00fal\u00e9rz\u00e9kenys\u00e9gi modellj\u00e9t adj\u00e1k. A cikk szerz\u0151i a Pfizer mRNS vakcina CARPA reakci\u00f3t kiv\u00e1lt\u00f3 hat\u00e1s\u00e1t vizsg\u00e1lt\u00e1k altatott sert\u00e9sen. A vakcin\u00e1t intrav\u00e9n\u00e1san, a hum\u00e1n d\u00f3zishoz k\u00e9pest 1, 2, \u00e9s 5-sz\u00f6r\u00f6s adagban adt\u00e1k, \u00e9s egy referens vegy\u00fclet, a zymosan hat\u00e1saival vetett\u00e9k \u00f6ssze. A v\u00e9rkering\u00e9si jellemz\u0151k alakul\u00e1s\u00e1t \u00e9s az EKG v\u00e1ltoz\u00e1sait k\u00f6vett\u00e9k. A v\u00e9rsejtsz\u00e1m v\u00e1ltoz\u00e1sait \u00e9s a komplement szint alakul\u00e1s\u00e1t a sert\u00e9sb\u0151l vett v\u00e9rmint\u00e1kban elemezt\u00e9k.<\/p>\n<p>A vizsg\u00e1lt 14 sert\u00e9sb\u0151l 6 esetben a zymosanhoz hasonl\u00f3 pulmon\u00e1lis v\u00e9rnyom\u00e1semelked\u00e9st, \u00e9s CARPA reakci\u00f3ra jellemz\u0151 v\u00e9rsejtsz\u00e1m (limfocita \u00e9s trombocita) v\u00e1ltoz\u00e1sokat tal\u00e1ltak. Egy \u00f6tsz\u00f6r\u00f6s d\u00f3zissal kezelt sert\u00e9sn\u00e9l k\u00f3ros EKG v\u00e1ltoz\u00e1s, \u00e9s \u00fajra\u00e9leszt\u00e9st ig\u00e9nyl\u0151 anafilaxi\u00e1s sokk alakult ki. In vitro vizsg\u00e1latokban a Pfizer-vakcina k\u00e9t d\u00f3zis\u00e1t 3 eg\u00e9szs\u00e9ges malac sz\u00e9rum\u00e1hoz adt\u00e1k, \u00e9s komplement feh\u00e9rj\u00e9k (C3a \u00e9s sC5b-9) termel\u0151d\u00e9s\u00e9t vizsg\u00e1lt\u00e1k. D\u00f3zisf\u00fcgg\u0151, a zymosanhoz hasonl\u00f3 komplement emel\u0151 hat\u00e1st tal\u00e1ltak.
