Ez\u00e9rt<a href=\"https:\/\/semmelweis.hu\/esk\/2025\/03\/11\/miben-kulonbozik-az-esk-uj-tipusu-surveillance-rendszer-a-hagyomanyos-leguti-figyeloszolgalattol\/\"> modern, innovat\u00edv, rugalmas, pontos surveillance-rendszerek kialak\u00edt\u00e1s\u00e1ra van sz\u00fcks\u00e9g a j\u00e1rv\u00e1ny\u00fcgyi d\u00f6nt\u00e9shozatal t\u00e1mogat\u00e1s\u00e1ra.<\/a><\/p>\n<p style=\"text-align: justify\"><strong>C\u00e9lunk, a l\u00e9g\u00fati k\u00f3rokoz\u00f3k<\/strong> (pl. SARS-CoV-2, influenza-, RSV \u2013 l\u00e9g\u00fati \u00f3ri\u00e1ssejtes v\u00edrus) \u00e1ltal okozott betegs\u00e9gek <strong>innovat\u00edv epidemiol\u00f3giai surveillance-rendszer\u00e9nek ki\u00e9p\u00edt\u00e9se \u00e9s m\u0171k\u00f6dtet\u00e9se, amely seg\u00edti a l\u00e9g\u00fati j\u00e1rv\u00e1nyok elleni v\u00e9dekez\u00e9st szolg\u00e1l\u00f3 inform\u00e1lt d\u00f6nt\u00e9shozatalt<\/strong>. Egy ilyen, \u00fajt\u00edpus\u00fa l\u00e9g\u00fati figyel\u0151szolg\u00e1lat alkalmas lehet arra, hogy el\u0151re jelezze a j\u00e1rv\u00e1nyok v\u00e1rhat\u00f3 lefut\u00e1s\u00e1t, a tet\u0151z\u00e9s\u00fck id\u0151pontj\u00e1t, valamint seg\u00edthet megv\u00e1laszolni olyan k\u00e9rd\u00e9seket, mint p\u00e9ld\u00e1ul: milyen terhel\u00e9s \u00e9ri az eg\u00e9szs\u00e9g\u00fcgyi ell\u00e1t\u00f3rendszert egy j\u00e1rv\u00e1ny alatt, mikor \u00e9s h\u00e1nyszor \u00e9rdemes beoltatnunk magunkat, illetve sz\u00fcks\u00e9g van-e tov\u00e1bbi int\u00e9zked\u00e9sekre (pl. kieg\u00e9sz\u00edt\u0151 maszkhaszn\u00e1latra).<\/p>\n<p style=\"text-align: justify\">Surveillance-rendszer\u00fcnk adatot szolg\u00e1ltat k\u00fcl\u00f6nb\u00f6z\u0151 m\u00f3dszerrel t\u00f6rt\u00e9n\u0151 <strong>v\u00e9d\u0151olt\u00e1s eredm\u00e9nyess\u00e9gi<\/strong> (VE) vizsg\u00e1latok kivitelez\u00e9s\u00e9hez is, vagyis annak becsl\u00e9s\u00e9re, hogy az adott v\u00e9d\u0151olt\u00e1s a val\u00f3 \u00e9letben, a hazai lakoss\u00e1g k\u00f6r\u00e9ben, a vizsg\u00e1lt l\u00e9g\u00fati szezonban milyen ar\u00e1nyban el\u0151zi meg az adott fert\u0151z\u0151 betegs\u00e9get. A v\u00e9d\u0151olt\u00e1sok val\u00f3 \u00e9letben t\u00f6rt\u00e9n\u0151 eredm\u00e9nyess\u00e9g\u00e9nek folyamatos monitoroz\u00e1sa a l\u00e9g\u00fati v\u00edrusok eset\u00e9ben az\u00e9rt is kiemelked\u0151en fontos, mert ezen v\u00edrusok genetikai szerkezete folyamatosan v\u00e1ltozik, \u00edgy a megszerzett v\u00e9detts\u00e9g \u201eelavul\u201d, illetve ereje a id\u0151vel cs\u00f6kken. Ennek k\u00f6vetkezt\u00e9ben id\u0151r\u0151l id\u0151re sz\u00fcks\u00e9gess\u00e9 v\u00e1lik a v\u00e9d\u0151olt\u00e1si strat\u00e9gia folyamatos adapt\u00e1l\u00e1sa, melyhez elengedhetetlen az aktu\u00e1lis epidemiol\u00f3giai helyzet \u00e9rt\u00e9kel\u00e9se. A rendszer tov\u00e1bbi el\u0151nye, hogy a cirkul\u00e1l\u00f3 v\u00edrusvari\u00e1nsok val\u00f3s idej\u0171 monitoroz\u00e1sa r\u00e9v\u00e9n megalapozottabb aj\u00e1nl\u00e1sok tehet\u0151k az olt\u00f3anyagok \u00f6sszet\u00e9tel\u00e9re vonatkoz\u00f3an.<\/p>\n<div class=\"responsive-tabs\">\n<h2 class=\"tabtitle\">B\u0151vebben<\/h2>\n<div class=\"tabcontent\">\n\n<h2><span style=\"text-transform: uppercase;text-align: justify\">L\u00e9g\u00fati v\u00edrusok elleni v\u00e9d\u0151olt\u00e1sok eredm\u00e9nyess\u00e9g\u00e9nek meghat\u00e1roz\u00e1sa Magyarorsz\u00e1gon a 2021-2028. \u00e9vekben<\/span><\/h2>\n<div class=\"w-100\" style=\"text-align: justify\">\n<p>Az SE ESK az Eg\u00e9szs\u00e9gbiztons\u00e1g Nemzeti Laborat\u00f3rium (ENL) RRF-2.3.1-21-2022-00006 azonos\u00edt\u00f3sz\u00e1m\u00fa projekt keret\u00e9ben a l\u00e9g\u00fati k\u00f3rokoz\u00f3k epidemiol\u00f3giai vizsg\u00e1lat\u00e1ra 2021-ben k\u00e9t, t\u00f6bbk\u00f6zpont\u00fa kutat\u00e1si h\u00e1l\u00f3zathoz csatlakozott, melynek c\u00e9lja kezdetben, a COVID-19 elleni vakcin\u00e1k forgalomba hozatal\u00e1t k\u00f6vet\u0151en e vakcin\u00e1k v\u00e9d\u0151olt\u00e1s-eredm\u00e9nyess\u00e9g\u00e9nek monitoroz\u00e1sa volt. Vizsg\u00e1latunkat k\u00e9s\u0151bb kiterjesztett\u00fck tov\u00e1bbi l\u00e9g\u00fati k\u00f3rokoz\u00f3k, pl. influenzav\u00edrus \u00e9s RSV okozta megbeteged\u00e9sek elleni v\u00e9d\u0151olt\u00e1sok eredm\u00e9nyess\u00e9g\u00e9nek meghat\u00e1roz\u00e1s\u00e1ra is. A \u201e<strong>L\u00e9g\u00fati v\u00edrusok elleni v\u00e9d\u0151olt\u00e1sok eredm\u00e9nyess\u00e9g\u00e9nek meghat\u00e1roz\u00e1sa Magyarorsz\u00e1gon a 2021-2028. \u00e9vekben\u201d<\/strong> elnevez\u00e9s\u0171, h\u00e1ziorvosok \u00e9s k\u00f3rh\u00e1zak bevon\u00e1s\u00e1val zajl\u00f3 epidemiol\u00f3giai vizsg\u00e1lat rendelkezik az Eg\u00e9szs\u00e9g\u00fcgyi Tudom\u00e1nyos Tan\u00e1cs Tudom\u00e1nyos \u00e9s Kutat\u00e1setikai Bizotts\u00e1g\u00e1nak szakmai-etikai enged\u00e9ly\u00e9vel.<\/p>\n<p>A kutat\u00e1s a VEBIS (Vaccine Effectiveness, Burden and Impact Studies) h\u00e1l\u00f3zat r\u00e9szek\u00e9nt val\u00f3sul meg, sz\u00e1mos eur\u00f3pai uni\u00f3s\/eur\u00f3pai gazdas\u00e1gi t\u00e9rs\u00e9gbeli tag\u00e1llam (pl. Franciaorsz\u00e1g, Portug\u00e1lia, Spanyolorsz\u00e1g, Hollandia) r\u00e9szv\u00e9tel\u00e9vel, valamint t\u00f6bb szervezet, k\u00f6zt\u00fck az ECDC, a WHO-EURO, region\u00e1lis \u00e9s nemzeti k\u00f6zeg\u00e9szs\u00e9g\u00fcgyi int\u00e9zetek, k\u00f3rh\u00e1zak, egyetemek, valamint kis- \u00e9s k\u00f6z\u00e9pv\u00e1llalkoz\u00e1sok egy\u00fcttm\u0171k\u00f6d\u00e9s\u00e9vel. A r\u00e9sztvev\u0151 orsz\u00e1gok az adatok \u00f6sszehasonl\u00edthat\u00f3s\u00e1g\u00e1nak \u00e9rdek\u00e9ben egys\u00e9ges protokollok szerint dolgoznak, ez\u00e1ltal nemcsak orsz\u00e1gos, hanem eur\u00f3pai szinten is meghat\u00e1rozhat\u00f3 a vakcin\u00e1k eredm\u00e9nyess\u00e9ge. Ez t\u00e1mogatja az uni\u00f3s szint\u0171 j\u00e1rv\u00e1ny\u00fcgyi d\u00f6nt\u00e9shozatalt \u00e9s az olt\u00e1si strat\u00e9gi\u00e1k kidolgoz\u00e1s\u00e1t.<\/p>\n<p>A VEBIS projektben a l\u00e9g\u00fati v\u00edrusokr\u00f3l \u00e9s a v\u00e9d\u0151olt\u00e1sok eredm\u00e9nyess\u00e9g\u00e9r\u0151l Eur\u00f3pa-szerte k\u00e9t f\u0151 ter\u00fcleten, egym\u00e1st\u00f3l f\u00fcggetlen\u00fcl zajlik az adatgy\u0171jt\u00e9s: egyr\u00e9szt<strong> a h\u00e1ziorvosi (alapell\u00e1t\u00e1si) szinten<\/strong>, m\u00e1sr\u00e9szt a <strong>k\u00f3rh\u00e1zi ell\u00e1t\u00e1sban<\/strong>.<\/p>\n<p>Magyarorsz\u00e1gon a <em>h\u00e1ziorvosok r\u00e9szv\u00e9tel\u00e9vel zajl\u00f3 vizsg\u00e1latba<\/em> a 6 h\u00f3napos \u00e9s id\u0151sebb lakoss\u00e1g k\u00f6r\u00e9b\u0151l, akut l\u00e9g\u00fati megbeteged\u00e9s (ARI)* t\u00fcnetei alapj\u00e1n meghat\u00e1rozott esetdefin\u00edci\u00f3 szerint v\u00e1logatjuk be a p\u00e1cienseket, m\u00edg <em>a k\u00f3rh\u00e1zi vizsg\u00e1latba <\/em>a s\u00falyos akut l\u00e9g\u00fati megbeteged\u00e9ssel (SARI)** \u00e9rkez\u0151 betegek ker\u00fclnek be a megfelel\u0151 t\u00fcnetek azonos\u00edt\u00e1sa alapj\u00e1n. A l\u00e9g\u00fati fert\u0151z\u00e9s k\u00f3rokoz\u00f3j\u00e1nak azonos\u00edt\u00e1s\u00e1ra laborat\u00f3riumi m\u00f3dszert (RT-PCR) haszn\u00e1lunk, mely alapj\u00e1n kimutathat\u00f3 az influenza-, a SARS\u2011CoV\u20112, az RS v\u00edrus, vagy egy\u00e9b k\u00f3rokoz\u00f3k jelenl\u00e9te. Ezut\u00e1n \u00f6sszehasonl\u00edtjuk az \u00e1toltotts\u00e1got a laborat\u00f3riumi vizsg\u00e1lattal meger\u0151s\u00edtett esetek \u00e9s a PCR-negat\u00edv kontrollok k\u00f6z\u00f6tt. A laborat\u00f3riumi krit\u00e9riumok szerint megfelel\u0151nek min\u0151s\u00edtett l\u00e9g\u00fati mint\u00e1kat ezt k\u00f6vet\u0151en tov\u00e1bbi szekven\u00e1l\u00e1sra k\u00fcldj\u00fck, ami lehet\u0151v\u00e9 teszi a k\u00f3rokoz\u00f3k genetikai jellemz\u00e9s\u00e9t.<\/p>\n<div class=\"fontos_div\">\n<p><em>* <strong>ARI-esetdefin\u00edci\u00f3:<\/strong>\u00a0A t\u00fcnetek hirtelen kezd\u0151dnek \u00e9s az al\u00e1bbi n\u00e9gy\u00a0l\u00e9g\u00fati t\u00fcnet\u00a0k\u00f6z\u00fcl legal\u00e1bb egy fenn\u00e1ll: k\u00f6h\u00f6g\u00e9s, torokf\u00e1j\u00e1s, l\u00e9gszomj, n\u00e1tha, tov\u00e1bb\u00e1 az orvos meg\u00edt\u00e9l\u00e9se szerint a t\u00fcnetek oka fert\u0151z\u00e9s.<\/em><\/p>\n<p><em>** <strong>SARI-esetdefin\u00edci\u00f3:<\/strong>\u00a0Legal\u00e1bb 24 \u00f3r\u00e1n kereszt\u00fcl k\u00f3rh\u00e1zi \u00e1pol\u00e1sra szorul\u00f3 p\u00e1ciens, akin\u00e9l az al\u00e1bbi l\u00e9g\u00fati t\u00fcnetek k\u00f6z\u00fcl legal\u00e1bb egy jelentkezik: l\u00e1z (\u2265 38\u00b0C), k\u00f6h\u00f6g\u00e9s, l\u00e9gszomj.<\/em><\/p>\n<\/div>\n<div class=\"keretes\">\n<p>Jelen kutat\u00e1sunk el\u0151zm\u00e9nye az <strong>I-MOVE (Influenza \u2013 Monitoring Vaccine Effectiveness in Europe) h\u00e1l\u00f3zat <\/strong>kutat\u00e1sa, amelynek c\u00e9lja az influenza elleni vakcina eredm\u00e9nyess\u00e9g\u00e9nek (VE) m\u00e9r\u00e9se Eur\u00f3p\u00e1ban. A projekt 2007-ben indult, \u00e9s a 2008-9-es influenzaszezon \u00f3ta multicentrikus (t\u00f6bbk\u00f6zpont\u00fa) \u00e9s egy\u00e9b vizsg\u00e1latokat v\u00e9gez, amelyekben Magyarorsz\u00e1gr\u00f3l a jelen kutat\u00e1st (VEBIS) v\u00e9gz\u0151 szakemberek a kezdetekt\u0151l fogva r\u00e9szt vettek.<\/p>\n<p>A jelenleg fut\u00f3 kutat\u00e1saink <em>els\u0151dleges c\u00e9lja<\/em> az <em>influenza<\/em> ill. a COVID-19 elleni v\u00e9d\u0151olt\u00e1sok \u00f6sszes\u00edtett, valamint v\u00e9d\u0151olt\u00e1st\u00edpus \u00e9s -m\u00e1rka szerint vizsg\u00e1lt eredm\u00e9nyess\u00e9g\u00e9nek meghat\u00e1roz\u00e1sa epidemiol\u00f3giai m\u00f3dszerrel a h\u00e1ziorvosi ill. k\u00f3rh\u00e1zi ell\u00e1t\u00e1st ig\u00e9nyl\u0151, laborat\u00f3riumilag igazoltan influenzav\u00edrus ill. SARS-CoV-2 okozta megbeteged\u00e9s megel\u0151z\u00e9s\u00e9ben a magyar lakoss\u00e1g k\u00f6r\u00e9ben. A kutat\u00e1s <em>h\u00e1ziorvosi \u00e1ga<\/em> a v\u00e9d\u0151olt\u00e1soknak a h\u00e1ziorvosi ell\u00e1t\u00e1sban jelentkez\u0151, k\u00f3rh\u00e1zi \u00e1pol\u00e1st nem ig\u00e9nyl\u0151 l\u00e9g\u00fati megbeteged\u00e9sek megel\u0151z\u00e9s\u00e9nek eredm\u00e9nyess\u00e9g\u00e9t, a <em>k\u00f3rh\u00e1zi \u00e1ga<\/em> pedig a s\u00falyos, 24 \u00f3r\u00e1n\u00e1l hosszabb k\u00f3rh\u00e1zi kezel\u00e9st ig\u00e9nyl\u0151 megbeteged\u00e9sek megel\u0151z\u00e9s\u00e9nek eredm\u00e9nyess\u00e9g\u00e9t becs\u00fcli meg. A h\u00e1ziorvosok \u00e9s a k\u00f3rh\u00e1zi szakemberek \u00e1ltal toborzott p\u00e1ciensekt\u0151l sz\u00e1rmaz\u00f3 l\u00e9g\u00fati mint\u00e1k PCR-vizsg\u00e1lat\u00e1t, valamint a mint\u00e1kb\u00f3l v\u00edrusok genetikai \u00e1llom\u00e1ny\u00e1nak kivon\u00e1s\u00e1t a Semmelweis Egyetem Laborat\u00f3riumi Medicina Int\u00e9zete v\u00e9gzi.<\/p>\n<p>A PCR-vizsg\u00e1lattal pozit\u00edv l\u00e9g\u00fati mint\u00e1kb\u00f3l kinyert v\u00edrusok \u00f6r\u00f6k\u00edt\u0151anyag\u00e1nak molekul\u00e1ris elemz\u00e9se alapj\u00e1n meg\u00e1llap\u00edthatjuk, hogy az adott v\u00edrus mely genetikai csoportba sorolhat\u00f3, illetve azonos\u00edthat\u00f3k az \u00faj v\u00e1ltozatok is. Ez seg\u00edt abban, hogy tov\u00e1bb pontos\u00edtsuk az ismert v\u00edrus\u00f6sszet\u00e9tel\u0171 vakcin\u00e1k eredm\u00e9nyess\u00e9g\u00e9nek \u00e9rt\u00e9kel\u00e9s\u00e9t aszerint, hogy <em>a vakcin\u00e1t a v\u00edrus mely vari\u00e1ns\u00e1val \/alt\u00edpus\u00e1val szembeni v\u00e9delem tekintet\u00e9ben \u00e9rt\u00e9kelj\u00fck<\/em>.
