The working group is the first of its kind enabling faster and more efficient diagnosis and treatment by a multidisciplinary approach. The diagnostic process will be accelerated, as patients will no longer need referrals to various specialists and all the required examinations take place at Semmelweis University.

“Pelvic pain can result from a variety diseases, including endometriosis, cystitis, chronic prostatitis in men and endometriosis in women, but neurological diseases can also be the cause of chronic pelvic pain” , explained Dr. Miklós Romics, specialist at the Department of Urology, founding member of the working group. He also mentioned that this is the first and only working group in Hungary focusing on pelvic pain.

Dr. Orsolya Oláh, gynecologist of the working group pointed out that chronic pelvic pain lasting longer than 6 months affects 10-15% of women above the age 25 and around one fifth of men. Dr. Oláh added that pelvic pain as an indication for diagnostic laparoscopy in gynecology accounts for 40% of the cases, and 60% of the these procedures reveal the pathological changes causing the symptoms. These can be problems with the organs, such as ovarian lesions, fibroids, endometriosis and adenomyosis. However, chronic pelvic inflammatory disease caused by chlamydia may also cause similar symptoms. These diseases are easy to identify, but there are other, less common conditions that are much more difficult to diagnose.

“Patients suffering from disorders that are difficult to diagnose have already undergone a lot of tests consequently, they might feel desperate and be in a bad mental state caused by many years of unsuccessful treatments. Therefore we also have a psychologist in the work group providing mental health support for patients.”, said Dr. Romics.

Often, it is hard to tell which area the pain originates, and often, the organ itself is not the source of the pain. Specialists often only focus on the symptoms and diseases related to their disciplines, but in certain cases, the cause of the pain may be originating from a different area. A urologist may attribute pain in the lower-abdomen and pelvic to problems with the bladder, but it may indicate gastrointestinal diseases and vice versa.

“It is acceptable not to have all the answers, but we should know which specialist to talk to when running out of diagnostic ideas”, added Dr. Romics.

The members of the special working group consists members who have thorough knowledge of pelvic pain in their field of specialty. The specialists include a gynaecologist (Dr. Orsolya Oláh), urologists (Dr. Miklós Romics, Attila Majoros PhD), a neurologist (Dr. Dániel Milanovich), a surgeon-proctologist (Dr. Péter Ónody), a radiologist (Dr. Bence Fejér), an anaesthesiologist (Dr. Kristóf Perczel), a psychologist (Dr. Éva Pintér) and a physiotherapist (Barbara Kovács). The multidisciplinary approach of the newly established work group will enable faster diagnosis for patients.

“Instead of having to visit 6-7 different doctors, the work group discuss cases in real time, so we are able to make decisions and propse the next step in the diagnostic and therapeutic process. All this is done at Semmelweis University making the assessment of imaging test results and communication between colleagues easier and faster”, added Dr. Miklós Romics.

The newly established working group is a great relief for patients who have been sent back and forth between doctors before. Once a patient is admitted by the working group the treating physician will know the patient’s detailed medical history right after the first visit, as it is shared in an online database.

“Therefore patients do not have to repeat their medical history at every consultation. This may even create a certain sense of security for the often disillusioned patients” – summarized dr. Orsolya Oláh.

The patient can contact the working group through any member and if the doctor is not sure about the medical solution or cannot identify the origin of the pain the patient’s records will be referred to other specialists within the working group.

The working group also intends to implement and develop new examination and treatment methods at Semmelweis University. For example, sacral neuromodulation, using a pacemaker implanted next to the spine, delivering a nonpainful electrical stimulus to alter the function of the sacral spine segment. This segment is responsible for the innervation of most of the pelvic organs, including the sensation of pain. With this intervention, a number of conditions associated with bowel and urinary problems, as well as pelvic pain can be treated.

Bernadett Bódi
Photo: Attila Kovács – Semmelweis University
Translation: Norbert Lukács

A medical group of nine specialists has been set up at Semmelweis University to effectively treat pelvic pain and related disfunctions.

[2

post-27690","post","type-post","status-publish","format-standard","has-post-thumbnail","hentry","category-news
["GET
